Procedures relating to admission, transfer and discharges of patients
The Hospital has a policy on admission, transfers and discharges of patients. This policy has been communicated and interpreted to all stakeholders.
Admissions
When a patient is admitted, she/he must be assessed by the doctors before admission, all the particulars of the patient must be clearly tabulated on patients chart, and consents should be obtained as an agreement for admission.
Transfer
Emergency patients when transferred to other hospitals for further management are escorted by trained people, relevant transport is arranged to suit the patient’s condition.
Discharges
Clinical evaluation of the patient is done by the doctor and if satisfactory, the patient is discharged; discharge summary must be available and must include the following:
